St. Cloud Rox Mascot Appearance Form

Please fill out the information below to submit a Mascot Appearance request. NOTE: Completion of this form is only a request and does not guarantee a mascot appearance. All requests must be within a 20 mile radius of the MAC and are limited to (1) one hour with a $60 fee. The normal fee is waived for schools and non-profits. Requests are recommended to be made at least (4) four weeks prior to the event. Contact the Rox for more information. Some private parties may not be accommodated, and all appearances are on a first-come, first-served basis. Chisel reserves the right to walk away from any appearance that he deems unsafe or harmful to the performer and/or guests (no refunds.)
